Medical SEO from our study of 78 Orange County practice websites: only 1 of 65 passed Google's mobile speed test. What slows patients down and the fix order.

A parent sits in a parking lot, phone in hand, typing "pediatrician accepting new patients near me." The first result starts to load. The screen stays white. By the time anything shows up, they've tapped back and picked someone else.

We wanted to know how often that happens, so we ran a medical SEO check on 78 health practice websites in Orange County, California: clinics, doctor offices, therapists, optometrists and physical therapy practices. The short version: the median practice homepage takes nine seconds to show its main content on a phone. Below is what we found, and the order we'd fix it in.

Quick answer: Medical SEO means getting a practice into the map pack, organic results and AI answers for "specialty + city" and condition searches, then turning that visit into a booked appointment. In our study of 78 Orange County health practices, the biggest gap was mobile speed: a median 9.0 seconds to show the main content, and only 1 of 65 sites passed.

What we checked

We took 78 health practice listings from OpenStreetMap in Orange County, California: 35 clinics, 30 doctor offices, 5 psychotherapists, 4 optometrists, 3 physical therapists and 1 other practice. Dentists have their own post.

We crawled every homepage on September 25, 2026, and ran each live one through Google PageSpeed Insights on mobile on September 26. Of the 78 listings, 65 sites were live and measurable. Eight (10.3%) were broken or gone: the patient clicks the website button on the map listing and gets an error.

The numbers below are a medical cut of our Orange County local business website study of 662 sites, so each one sits next to the figure for all local businesses.

Largest Contentful Paint, or LCP, is how long the biggest thing on the screen, usually the hero image or headline, takes to appear. Google calls under 2.5 seconds good. Medical practice homepages took a median 9.0 seconds, the slowest of the groups we cut from the study.

We saw the same causes over and over. Full-width hero sliders that load several large photos at once. Photos straight from a camera, never compressed. Patient portal widgets and chat scripts loading before the page can be read. And a stack of third-party tags, sometimes five or six on one page, each adding its own delay. Booking and calls: half right

More than half of the practices let patients book online, but one in five has no tap-to-call link on mobile.

  • 57% offer online booking, against 38% of all local businesses.
  • 20% have no tap-to-call link, so a patient has to copy the number by hand.
  • 66% are what we call lead-ready: HTTPS, a mobile layout, tap-to-call and a form or booking. Across all local businesses it's 59%, so a third of practices still miss at least one basic.

The fix doesn't need a redesign. Put a tap-to-call button in the mobile header. Put the booking link above the fold, not in a dropdown. And if you have after-hours instructions, a nurse line or an urgent care referral, say so on the page instead of leaving a worried patient guessing.

The signals Google and AI use

Most practices give Google and AI assistants less to work with than they could.

Local business structured data tells Google in code what the practice is, where it is and when it's open. Analytics tells you which searches bring patients. Without either, healthcare SEO is guesswork.

The copyright line looks trivial, but a footer that still says 2013 tells every visitor, and every reviewer at Google, that nobody has touched the site in years.

Healthcare SEO content that ranks and stays compliant

Healthcare content has to rank and stay inside the lines at the same time.

Build one page per service and one page per condition you treat, not one "Services" page. Have each page written or reviewed by a named clinician with credentials shown. Google's quality guidelines weigh experience, expertise, authoritativeness and trust (E-E-A-T) most heavily on health topics, which it treats as "your money or your life" content. An insurance-accepted page and clear location and hours answer what patients need before they call.

A few rules matter more here than in any other industry:

  • Don't collect health details through a contact form that isn't set up for HIPAA-appropriate handling. Ask for name, phone and preferred time, and leave the medical history for the intake process.
  • Don't publish testimonials, reviews or before-and-after photos without the patient's written authorization.
  • Don't promise outcomes.

Google Business Profile for practices

A practice's Google Business Profile often gets more views than its website, so it deserves the same care.

Create one profile per physical location. Decide whether individual doctors need their own practitioner profiles; Google's guidelines for representing your business explain when that's allowed. Choose accurate categories, keep holiday hours current, add a direct appointment link and use real photos of the office and staff.

When you reply to reviews, don't confirm or deny that the reviewer is a patient, even if they wrote it themselves. Keep replies general and invite them to call the office. Our Google Business Profile optimization guide covers the rest of the setup.

What practice owners ask before hiring

We read what practice owners ask each other on Reddit's SEO and marketing forums. Three themes keep coming back.

  • Paying for maintenance, not growth. A solo specialist described paying an industry website vendor about $250 a month just to keep the site running, with no growth work included.

Based on what we found, this is the order we'd use:

  1. Fix speed first. Resize and compress hero images, drop autoplay sliders on mobile, and load chat and portal scripts after the main content.
  2. Add a tap-to-call button to the mobile header.
  3. Move the booking link above the fold and add after-hours instructions.
  4. Add LocalBusiness or MedicalBusiness schema to every location page.
  5. Build one page per service and condition, reviewed by a named clinician.
  6. Complete Google Business Profile for each location, with hours, appointment link and photos.
  7. Ask every patient for a review with a simple link, and reply without confirming patient status.
  8. Install analytics and call tracking so you can see which searches bring appointments.
  9. Add alt text and update stale pages, including the footer year.
  10. Decide on AI crawler access on purpose, then check the firewall matches that decision.
